<p><strong>Your Role:</strong><br>As a vital member of our interdisciplinary team, you’ll work under the supervision of a Clinical Manager to coordinate all aspects of patient care—ensuring individualized, compassionate support that maximizes quality of life.</p>

<p><strong>Key Responsibilities:</strong></p>

<ul>

<li>Conduct ongoing, holistic assessments of patients and families</li>

<li>Develop and manage comprehensive care plans in coordination with the care team</li>

<li>Facilitate interdisciplinary case conferences and care planning discussions</li>

<li>Collaborate with physicians, hospice staff, volunteers, and external agencies to ensure optimal support</li>

<li>Advocate for patients and families, addressing physical, emotional, and psychosocial needs</li>

</ul>

<p><strong>What You’ll Bring:</strong></p>

<ul>

<li>Active Massachusetts RN license in good standing</li>

<li>Associate’s degree in Nursing required; BSN or equivalent preferred</li>

<li>Proven ability to provide care with compassion and cultural sensitivity in diverse home or facility settings</li>

<li>Strong interpersonal skills and the ability to thrive in a collaborative team environment</li>

<li>Flexibility, self-direction, and sound clinical judgment</li>

<li>Valid driver’s license and access to an insured, reliable vehicle</li>
