LPN (Licensed Practical Nurse)

<p><strong>POSITION SUMMARY:</strong><br>Provide personal care, assist in recreational activities, perform assessments, and give companionship to the residents of the Abe’s Garden.</p><p><strong>ROLES AND RESPONSIBILITIES:</strong><br>Direct and supervise the Care Partners working within Abe’s Garden, and provide training and guidance as needed.<br>Observe, record, and report resident behaviors, symptoms, or changes in resident’s record and to oncoming shift.<br>Perform physical assessments and obtain/maintain resident health histories.<br>Record resident medical information and vital signs, note changes, and pursue more specific investigation as needed.<br>Administer medications and other treatments to aid various conditions.<br>Keep written documentation of services provided, as well as communication about high risk behaviors and/or critical incidents.<br>Respond to and treat resident emergencies.<br>Educate residents and their families about their conditions while establishing trust and rapport with them.<br>Provide companionship and emotional support to residents and their families.<br>On an as-needed basis, assist residents with daily activities, including but not limited to bathing, grooming, toileting, eating, dressing and engagement activities.<br>Act as liaison between resident, family, physician and ancillary services<br>Maintain continuing education necessary for keeping required credentials.<br>Maintain regular and timely attendance.<br>Abe's Garden is an equal opportunity employer and a drug-free workplace.</p>
