<strong>About the Position:</strong>
The <strong>Clinical Consultant for the Enhanced Care Management (ECM) - Community Supports (CS)</strong> program supports the program and Advocates by assuring the quality of clinical service is being maintained. Ensures that clinical records are accurate, current, and meet Medical Assistance standards and regulations. The Clinical Consultant provides expertise and guidance to improve student care, program effectiveness, and clinical outcomes. Strong clinical background, experience in care management, and a passion for transforming service delivery through innovative practices. ECM is a comprehensive, patient-centered approach to healthcare that aims to improve outcomes for high-need populations. This involves the coordination of medical, behavioral, and social services to address the full range of student needs, focusing on those with complex and chronic conditions. ECM programs are designed to optimize care delivery, reduce unnecessary hospitalizations, and improve the overall quality of life for students.

<p>The Enhanced Care Management (ECM) Clinician is responsible for member coordination, improving health outcomes, enhancing satisfaction, and reducing unnecessary healthcare utilization. ECM is a comprehensive, patient-centered approach to healthcare that aims to improve outcomes for high-need populations. This involves the coordination of medical, behavioral, and social services to address the full range of patient needs, focusing on those with complex and chronic conditions. ECM programs are designed to optimize care delivery, reduce unnecessary hospitalizations, and improve the overall quality of life for. The intention of our services is to help people who have not been able to get the needed help in terms of Housing, Mental Health, Substance Abuse treatment, and linkages to community resources to help minimize recidivism. </p>

<p>The role of The ECM Clinical Consultant provides clinical oversight, consultation, and quality assurance across multiple Enhanced Care Management (ECM) contracts spanning various Managed Care Plans and counties. This role is non-direct service and does not carry an individual caseload. The Clinical Consultant ensures clinical integrity, compliance, and quality of ECM services by reviewing and approving assessments and care plans, offering clinical guidance to staff, and supporting treatment-related decision-making.</p>

<p>The ECM Clinical Consultant plays a key role in ensuring services align with CalAIM/ECM requirements, evidence-based practices, and trauma-informed care principles, particularly for high-acuity populations, including justice-involved individuals.</p>
<p>This position is primarily office-based and telehealth-driven, with occasional travel required to support program implementation, training, and stakeholder engagement.</p>

<p><strong>Essential Duties:</strong></p>
<li>Review, approve, and provide clinical sign-off on comprehensive assessments, individualized care plans, and treatment-related documentation </li>
<li>Ensure documentation meets medical necessity and ECM/CalAIM standards </li>
<li>Provide feedback to staff to improve clinical quality, accuracy, and compliance </li>
<li>Maintain consistency in clinical practices across multiple contracts, counties, and Managed Care Plans </li>
<li>Serve as a clinical resource and consultant to ECM staff and multidisciplinary teams </li>
<li>Provide guidance on complex and high-risk cases, behavioral health and substance use concerns, crisis intervention and stabilization, and treatment planning and care coordination strategies </li>
<li>Provide consultation and support to staff, including availability for case debriefing and clinical guidance </li>
<li>Assist with cases requiring escalated clinical involvement </li>
<li>Provide consultation on both clinical and administrative issues as requested </li>
<li>Assist in the development and implementation of clinical strategies aligned with program goals </li>
<li>Support the integration of evidence-based practices into ECM service delivery </li>
<li>Evaluate and recommend clinical models, care pathways, and best practices to enhance outcomes </li>
<li>Collaborate with Program Director and leadership to support program design and clinical effectiveness </li>
<li>Support the implementation of Teaching and Therapeutic Community models within programming </li>
<li>Provide oversight to ensure ECM services meet local, state, and federal regulations, Managed Care Plan requirements, and contractual expectations and deliverables </li>
<li>Conduct chart reviews, audits, and quality checks </li>
<li>Identify gaps, trends, and risks in clinical service delivery and recommend improvements </li>
<li>Support audit readiness, site visits, and compliance reviews </li>
<li>Develop and deliver training on clinical documentation standards, care coordination techniques, student engagement strategies, and best practices in ECM service delivery </li>
<li>Support onboarding and ongoing staff development and clinical competency building </li>
<li>Promote consistency in clinical approaches across programs and counties </li>
<li>Serve as a clinical liaison between ECM programs, healthcare providers, and community partners </li>
<li>Collaborate with multidisciplinary teams to coordinate care and improve service delivery </li>
<li>Participate in committees and meetings to advocate for ECM services and the populations served </li>
<li>Support quality improvement initiatives in partnership with leadership and care teams </li>
<li>Provide clinical supervision and guidance to ECM Advocates and Housing Navigators (as required for clinical oversight) </li>
<li>Offer ongoing consultation, mentorship, and support without direct managerial responsibility </li>
<li>Support staff in applying trauma-informed care and client-centered approaches </li>
<li>Attend meetings, trainings, and workshops as requested </li>
<li>Maintain documentation of clinical reviews and approvals </li>
<li>Provide reports on clinical trends, outcomes, and recommendations </li>
<li>Perform other duties as assigned </li>
What You Will Bring:
<p><strong>Required:</strong></p>
<ul>
<li>Master’s or Doctoral degree in behavioral health, public health, or related human services field </li>
<ul>
<li>Active, unrestricted licensure in California: </li>
<ul>
<li>LCSW, LMFT, LPCC, PsyD, or PhD</li>
<li>Minimum 5 years of clinical experience, including: </li>
<ul>
<ul>
<li>Behavioral health (mental health and/or substance use) </li>
<li>High-acuity or underserved populations (e.g., homeless, justice-involved) </li>
<ul>
<li>At least 2 years of experience in care management, population health, or ECM-related services </li>
