Field Case Manager - LPN

<p><strong>DUTIES AND RESPONSIBILITIES:</strong></p><p>Ensures the quality and safe delivery of home health services:</p><ul><li>Completes nursing assessment forms promptly per agency policy and procedures;</li><li>Supervises Home Health Aides every two weeks;</li><li>Identifies physical, emotional and developmental needs of the clients;</li><li>Develops individual nursing care plan reflects current client needs before case conference;</li><li>Provides health teachings to clients and their families;</li><li>Makes proper and accurate clinical record documentation that indicates delivery and safe home health care services.</li></ul><p>Implements current practice:</p><ul><li>Assesses client condition on every home visit;</li><li>Communicates client assessments to appropriate individual concerned;</li><li>Implements nursing interventions to meet client needs and changed conditions;</li><li>Electronic Documentation of accurate clinical record of every home visits per agency policy and procedures, requirements</li><li>Accurately transcribes, implements and evaluates the physician’s plan of care promptly;</li><li>Implements infection control and safety measures per agency policy and procedures;</li><li>Monitors client for compliance with defined care plans.</li></ul><p>Performs assigned treatment/skilled procedures but are not limited to the following:</p><ul><li>Wound and PICC line care;</li><li>Insert and irrigate Urinary Catheters</li><li>Suction and cleans tracheal tubes;</li><li>Draws blood for laboratory testing.</li><li>Complies with home health agency’s policy and procedures manual.</li><li>Complies with federal, state and local regulatory agencies.</li></ul>

<p>&nbsp;</p><ul><li>A graduate of an accredited school of nursing.</li><li>Must be a Licensed Practical Nurse and passed the state board.</li><li>Has experience in home health or public health nursing preferred but nor required.</li><li>Knowledgeable about current nursing trends and legislative issues.</li><li>Must possess excellent verbal and written communication skills.</li><li>Excellent administrative, organization and leadership skills.</li></ul><p><strong>PHYSICAL REQUIREMENTS:</strong></p><ul><li>Visual/hearing ability sufficient to comprehend written/verbal communication.</li><li>Ability to perform tasks involving physical activity which may include heavy lifting and extensive bending and standing.</li><li>Ability to deal effectively with stress.</li></ul>
