<p>Position Summary</p>

<p><u>Clinical Pharmacist</u></p>

<p>Baldwin Family Health Care is seeking a passionate and patient-centered Clinical Pharmacist to join our growing healthcare team. This unique opportunity combines direct patient care, chronic disease management, interdisciplinary collaboration, and pharmacy residency leadership.</p>

<p>The Clinical Pharmacist will work closely with providers and care team members to optimize medication therapy and improve patient outcomes through evidence-based pharmaceutical care. The successful candidate will also serve as the primary preceptor for the PGY1 Community-Based Pharmacy Residency Program's Patient Care Learning Experience and function as the Residency Program Coordinator (RPC).</p>

<p><u>Key Responsibilities</u></p>

<p>Clinical Pharmacy Services</p>

<ul>

<li>Collaborate with providers and healthcare staff to deliver evidence-based, cost-effective medication management.</li>

<li>Conduct comprehensive medication reviews and medication reconciliation.</li>

<li>Provide patient education on medications, disease states, lifestyle modifications, and self-management strategies.</li>

<li>Identify, resolve, and prevent medication-related problems.</li>

<li>Respond to drug information questions from providers and staff.</li>

<li>Monitor laboratory values and clinical outcomes to optimize medication therapy.</li>

</ul>

<p>Ambulatory Care &amp; Chronic Disease Management</p>

<p>Under Collaborative Practice Agreements, manage patients with chronic conditions including:</p>

<ul>

<li>Diabetes</li>

<li>Hypertension</li>

<li>Hyperlipidemia</li>

<li>Anticoagulation</li>

<li>Chronic Obstructive Pulmonary Disease (COPD)</li>

<li>Tobacco Use Disorder</li>

</ul>

<p>Program Development &amp; Quality Improvement</p>

<ul>

<li>Assist in the development, implementation, and evaluation of clinical pharmacy services.</li>

<li>Participate in quality improvement and population health initiatives.</li>

<li>Support medication access programs and cost-containment strategies, including 340B opportunities.</li>

</ul>

<p>Residency Program Leadership</p>

<ul>

<li>Serve as Residency Program Coordinator for the PGY1 Community-Based Pharmacy Residency Program.</li>

<li>Act as primary preceptor for the Patient Care Learning Experience.</li>

<li>Mentor pharmacy residents and contribute to residency program development and accreditation activities.</li>

<li>Promote a culture of teaching, learning, and professional growth.</li>

</ul>

<p><u>Qualifications Required</u></p>

<ul>

<li>Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.</li>

<li>Licensed Pharmacist in the State of Michigan or eligible for licensure.</li>

<li>Outpatient, community, ambulatory care, or retail pharmacy experience.</li>

<li>Strong communication, organization, and interpersonal skills.</li>

<li>Ability to work effectively in a multidisciplinary team environment.</li>

<li>Commitment to serving diverse and underserved patient populations.&nbsp;</li>

</ul>

<ul>

<li>Completion of a PGY1 Pharmacy Residency, and/or ambulatory care practice experience.</li>

</ul>

<p>Preferred</p>

<ul>

<li>Experience working within a Federally Qualified Health Center (FQHC) or similar safety-net healthcare setting.</li>

<li>Experience with the 340B Drug Pricing Program.</li>

<li>Familiarity with PioneerRx and NextGen electronic health record systems.</li>
